Fellowship: Year I
Components of the first year of training include:
CLINICAL ROTATION
The fellow is responsible, together with an attending, for the provision
of clinical consultative services for all inpatients, surgical patients,
and emergencies over a one-month period and for the teaching of students
and residents.
OUTPATIENTS
Outpatients are seen daily, and the fellow will participate in the evaluation
of these patients, together with the attendings, sharing in the evaluation
of the noninvasive testing and in the formulation of management plans. Fellows
are assigned to clinic days on a rotational basis throughout the year. Additionally,
fellows have own continuity clinic where long-term follow-ups are conducted.
INTENSIVE CARE SKILLS
While on the clinical rotation, the fellow will spend time in the Neonatal
and Pediatric Intensive Care Units. They will share in the management of
the postoperative cardiac patients and of the acutely ill neonate with heart
disease. Daily morning rounds are made in the PICU together with the intensivists
and cardiac surgeons.
ELECTROPHYSIOLOGY
Two rotations on the Electrophysiology Service for one month in the first
and second year takes place in conjunction with the electrophysiology program
based in Jacksonville.
PATHOLOGY
Monthly didactic pathology sessions are held within the division throughout
the year. Elective periods of one month are conducted with Dr. Van Mierop
to provide an in depth review of cardiac embryology and anatomy.
NONINVASIVE TESTING
A newly constructed, fully equipped Heart Station is the site for training
of fellows in the interpretation of electrocardiograms and of M-mode and
two-dimensional echocardiography with Doppler, and color-Doppler, a cornerstone
of modern pediatric cardiology diagnosis. The fellow is trained not only
to interpret, but to carry out high-quality studies using these techniques.
Fetal echocardiography, and transesophageal echocardiography are also conducted
in a new facility within the 11th floor cardiac function and imaging center.
The laboratory performs approximately 5,000 echocardiograms annually. Holter
monitoring, exercise stress testing, and tilt-table testing are also included.
Magnetic resonance imaging is also available in the Department of Radiology
and Center for Structural Biology. Echocardiography conference is held once
a week and there is a graphics conference and part of the weekly patient
presentation conference.
CATHETERIZATION LABORATORY
Training in cardiac catheterization techniques, angiography and interventional
cardiology is a major part of the training program. The fellow is expected
to gradually become proficient and then expert in cardiac catheterization
and data collection, its interpretation and significance. The fellow will
be exposed to a variety of interventional techniques, such as pulmonary and
aortic valvuloplasty, septostomy, stent and coil placement, pressure-volume
assessment and endomyocardial biopsy. The training in the cardiac catheterization
laboratory is enhanced by pre-catheterization and post-catheterization conferences
and weekly patient-management conferences.
