Discharge Coordinators
The goals of the discharge coordinator are to work with the primary medical team to ensure that infants requiring complex medical care at home have a smooth transition from the NICU to their home environment. This involves the teaching of parents and/or primary care givers how to care for infants who may require ventilatory support, home oxygen therapy, home monitoring, and complex administering of medications. The discharge coordinator also assists in the teaching on how to properly care for G-tubes, ostomies, and central lines, and how to correctly use feeding pumps. The arrangement of home equipment including oxygen for travel out of state using different airlines and home nursing services are also provided.
In addition, the discharge coordinator oversees that proper administration of Synagis is provided to high-risk neonates and that at-risk neonates receive proper ROP screening. CPR teaching is provided to high-risk neonates, especially those going home on monitoring equipment. Follow-up appointments with our neonatal follow-up clinic are also arranged by the discharge coordinator.
